Dosimeter Badge Service: Terms and Conditions
1. Introduction
Welcome to Newvetequipment.com Dosimeter Badge Service. By subscribing to our service, you agree to comply with and be bound by the following terms and conditions. Please read these terms carefully before using our service.
2. Service Description
The dosimeter badge service provided by Newvetequipment.com includes the annual rental, monitoring, and reporting of radiation exposure through personal dosimeter badges. The service is designed to ensure that individuals who work in environments with radiation exposure maintain safe levels.
3. Subscription and Fees
3.1 Fees: Our dosimeter badge service has a cost of $88 per person/per year. Shipping costs are covered and all fees are non-refundable unless otherwise stated.
3.2 Late/ Unreturned Fee: The badges must be returned quarterly to ensure accurate data on exposure levels and prompt intervention if needed. If the badges are not returned within 60 days after the end of the wear period, a late fee of $50 for every badge not returned will be charged.
3.3 Payment Terms: Payment is effective once the order is placed in newvetequipment.com
3.4 Subscription: By subscribing to our service, you agree to pay the subscription fees as outlined in your service agreement. Subscription terms are annually and it’s automatically renewed once the yearly subscription ends.
4. Dosimeter Badge Usage
4.1 Proper Use: Subscribers agree to use the dosimeter badges only for their intended purpose, which is to monitor radiation exposure. Any misuse or tampering with the badges is strictly prohibited.
4.2 Badge Assignment: Badges are assigned to specific individuals and should not be transferred or shared. Each veterinary dosimeter badge is person-specific. Badges cannot be transferred from a facility to another. If an employee is transferred to another facility, they will need to order a new badge.
4.3 Badge Return: Badges must be returned to Radiation Detection Company (RDC) at the end of every quarterly period. Unreturned badges will incur a replacement fee of $50 per person/per badge.
5. Monitoring and Reporting
5.1 Monitoring: Radiation Detection Company (RDC) will monitor the radiation exposure recorded by the dosimeter badges and provide quarterly reports to subscribers.
5.2 Reports: Reports on radiation exposure will be sent to the subscriber according to the schedule agreed. Also can be downloaded from your online account management portal https://myradcare.radetco.com/signin.aspx
6. Subscriber Responsibilities
6.1 Compliance: Subscribers are responsible for complying with all relevant regulations and guidelines regarding radiation exposure in their industry.
6.2 Accuracy: Subscribers must provide accurate information about the individuals using the badges and ensure that the badges are worn correctly at all times.
7. Liability
7.1 Limitation of Liability: Newvetequipment.com is not liable for any damages or losses resulting from the improper use of dosimeter badges or failure to comply with safety guidelines.
7.2 Indemnification: Subscribers agree to hold harmless Newvetequipment.com from any claims, damages, or expenses arising out of their use of the dosimeter badge service.
8. Termination
8.1 Termination by Subscriber: Subscribers may terminate the service by providing [30 days] written notice. Fees for the current billing cycle will not be refunded. Subscribers must notify via email to newvetequipment.com to inactivate any badge at any time and the badge must be returned within 60 days after the end date of wear period to avoid unreturned charges.
8.2 Termination by Newvetequipment.com: Newvetequipment.com reserves the right to terminate the service for any subscriber who fails to comply with these terms and conditions.
9. Amendments
Newvetequipment.com reserves the right to amend these terms and conditions at any time. Subscribers will be notified of any changes via email or through the subscriber portal.
10. Governing Law
These terms and conditions are governed by NVLAP accreditation.
